I went hunting for something in my spare room and I found the old PS2 and SSX Tricky so that's me gone for weeks.

Nope - all teams get the points for a valid item - the ones where there is grading are as stated i.e. the holiday souvenirs will be given points in order of hideousness, 6 for the most vile then 5, 4, 3, 2 and 1 point for the one that's just a bit off.

Yes, that's fine - not sure why you would need both sides of the record but yes, no problem.

The rules state: "The book must state which award it has won on its front or back cover" - as an eBook is a digital file and therefore doesn't have such a thing as a cover, it would not be eligible.
